Eligibility

Eligibility for specific services is determined by a care manager during an in-home assessment. PASSPORT serves individuals who are not eligible for services through MyCare Ohio. Open to residents of Hamilton and Clermont Counties who: -are age 60 or older -qualify for Medicaid -require skilled or intermediate assistance with activities of daily living (nursing home level care)
