Eligibility

Residents of Ashtabula, Columbiana, Mahoning, and Trumbull Counties age 60 and older who meet income requirements and who require assistance with certain activities of daily living. Eligibility for specific services is determined by a care manager during an in-home assessment.

Description

Offers assessment and case management for older adults to assist them with remaining in their home as long as possible. Through case management services, participants have access to a variety of in-home services, such as assistance with chores, home delivered meals, accompaniment to and from medical appointments, personal care assistance, etc.
