Application to the program is free. Premiums may be charged for the medical coverage.
Eligibility
Serves residents of Geauga County age 16 and older who are U.S. citizens or qualified aliens. Must be determined disabled by the Social Security Administration or by Ohio Medicaid. Must be employed in paid, taxed work (full- or part-time). Must meet certain financial criteria (there are a number of deductions, so anyone who may otherwise be eligible is encouraged to apply). Premiums are required for those with annual gross income greater that 150% of Federal Poverty Level (FPL). Consumers who must pay premium will be subject to termination from the program if they do not pay the premium for two consecutive months. However, they may re-enroll once all delinquent premiums are paid (as long as they meet all other eligibility).
