Fixed Fee (low cost): Blood Glucose Strips, No Coding Box of 50: $15 per box; Blood Glucose Monitor: One free monitor annually; Insulin Syringes: $13 for a box of 100; Lancets: box of 100 $5 per box with a minimum order of 2 boxes (minimum order $10)

Open to those with household income at or below 300% of federal poverty level. They may be insured, uninsured, or simply have inadequate prescription coverage. May be enrolled in Medicare Part D.
