Description

Helps assure that consumers of long-term care services make the best decisions possible based on the most complete information available. Provides consumers with nursing facility survey information from the Ohio Department of Health, as well as verified complaint activity by Ombudsman investigators. Lists of all nursing homes, assisted living, and group, family and foster homes in the Summit, Portage, Stark and Wayne counties are also available.

Direction Home Akron Canton Area Agency On Aging And Disabilities

Plans, funds, and coordinates services for older persons and persons with disabilities in Portage, Stark, Summit, and Wayne counties to live with dignity and choices in their homes and communities for as long as possible. Administers PASSPORT, MyCare Ohio, The Assisted Living Waiver, The Ohio Home Care Waiver, Care Coordination, Elder Rights, Family Caregiver Support Program, Nursing Home Placement Assistance Program, and other services designed to help older adults and individuals with functional limitations maintain independence. Provides funds for services administered by other community agencies including client finding, specialized information and referral, transportation, home-delivered meals, medical treatment, personal care, respite care, safety monitoring, health assessments, congregate meals, recreational activities, and adult day care.
