An application (BMV Form 1152), proof of valid insurance and an initial $25 payment may be submitted by mail, or in person at a Deputy Registrar Office (for an additional $10 fee). - The plan will allow individuals owing $150 or more in reinstatement fee to become valid or eligible to retest for a driver license by paying only $50 or more every 30 days for as long as it takes to pay their reinstatement fees. - Applicants submitting at a Deputy Registrar Office will be charged an additional $10 service fee with the application. Once approved for the fee installment plan, a minimum payment of $25 will be due every 30 days from the last payment. A new payment coupon will be mailed upon receipt of payment. Payment options are: - Pay online with a credit card by visiting www.bmv.ohio.gov/susp-fees-payment.aspx and click on the "pay your reinstatement fees online" link. - In person at any Deputy Registrar License Agency by check, cash, money order or credit card. Customers who meet the following requirements are eligible to participate in the installment plan program: - Must owe at least $150 in reinstatement fees. - Must have met all other reinstatement requirements except for paying reinstatement fees. - Must show current proof of insurance. - Must not be under suspension and have no pending suspensions. - Must not be currently on a court-ordered payment plan. NOTE: The payment plan will be canceled if the individual does not make at least one $25 payment every 30 days or if they become suspended while on the payment plan.
