Individuals with NO or LOW income who need a birth certificate for employment, housing or education purposes. Cannot have received one before. Must have a referral from an agency with name, state and date of birth for the person in need and reason for need. Send referrals to: [email protected]
Requires a referral letter, on letterhead, from the requesting organization or agency, stating a birth certificate is required. Include name of the person to receive the birth certificate, their date of birth and state of birth. Service can be provided one per person and two per household only. Each state will have further document requirements.
Mon-Fri 9:30 am-1:30 pm, except holidays. Appointment needed.

Referral is required. Referral must be emailed to [email protected], and must inclue: letter on referring agency's letterhead , name of recipient, date of birth of recipient, state of birth of recipient. If recipient is a minor, parent/guardian name date and state of birth. Must also have reason for need (i.e. housing, employment, school etc.) , and be signed and dated by referring professional.
