Application process

Individuals may apply at the Neighborhood Family Service Center (NFSC) serving their ZIP code. Persons who are homeless can receive initial service at any NFSC. Individuals should phone to locate their NFSC 24 hours a day, 7 days a week. After determining the appropriate Center, may walk-in to get an application Monday through Friday from 8:30am to 4:30pm. Applications are also available 24 hours a day, 7 days a week on the web site. NOTE: The Ohio Benefits system has a scheduled outage on Friday December 3rd, 2021. JFS wll be unable to process applications or reported changes on TANF, SNAP, and Medicaid cases. Any phone calls received regarding case status changes or case inquiries will not be able to be answered during this period. Any verifications or applications received will not be processed until Monday December 6th, 2021 at the earliest. Applications will be reviewed for potential expedited services.

Eligibility

- Serves residents of Carroll County age 16 to 64 who are U.S. citizens or qualified aliens. - Must be determined disabled by the Social Security Administration or by Ohio Medicaid. - Must be employed in paid, taxed work (full- or part-time). - Must meet certain financial criteria (there are a number of deductions, so anyone who may otherwise be eligible is encouraged to apply). - Premiums are required for those with annual gross income greater that 150% of Federal Poverty Level (FPL). Consumers who must pay premium will be subject to termination from the program if they do not pay the premium for two consecutive months. However, they may re-enroll once all delinquent premiums are paid (as long as they meet all other eligibility).
