Application process

To receive assistance, please schedule an appointment by emailing [email protected] or calling 740-200-0689. If you leave a voicemail, please be sure to include your name and the best way to reach you. Someone from the Care Team will respond to your inquiry as soon as possible. Adults served must bring a copy of an official Ohio ID, such as a driver's license or other official ID. Some programs may require additional documentation. Your Care Team representative will inform you if other documents are required.

Eligibility

To receive services, you must be an Ohio resident. Some programs have income eligibility guidelines. If you need assistance, please contact us at 740-200-0689 or email [email protected]. Serves people living in Knox County and surrounding rural communities in Licking, Coshocton, Muskingum, and Holmes Counties.

Languages

English is our primary language. We do not always have an interpreter on site for every language spoken, but we will work to accommodate any non-English speaking individual.
