Application process

- Jefferson County Job and Family Services receives a monthly list from Children and Family Services of those aging out of foster care, documents the clients' unique status, and opens the Medicaid case. - If someone meets criteria, is not currently receiving coverage, and wishes to apply, they should be clear about their history as a foster child when applying. - This should prompt the eligibility worker to explore the coverage. Call Monday through Friday from 8:30am to 4:30pm to schedule an appointment. Initial application may also be submitted through web site (once application is received, client will be contacted within 5 business days to schedule an appointment). An intake interview is necessary in order to obtain service (Homebound individuals may conduct the interview by phone or send an authorized representative in their place. Call for details). Clients who may be eligible for Medicare will be referred for enrollment. MCP-Eligible Clients: - Must select and enroll in an MCP or will be auto enrolled randomly (plan can be changed with reason or during open enrollment). NOTE: Those who are waiting for disability determination by Social Security can receive their disability determination for Medicaid through the county - the shortest time for that determination would be three (3) months, but more commonly this takes five (5) to six (6) months.

Fee

No fee to apply. Once receiving coverage, individual MCPs may charge co-payments for some services.

Eligibility

- Serves those age 18 through 26 who have aged out of foster care and who reside in Jefferson County. - Those who age out of foster care will be covered through age 26 and are eligible to start coverage at any point before age 26. - There are no income or asset requirements for this group. - Those who aged out of foster care in other states, and who meet these guidelines are eligible for coverage as part of this group. - Those who left foster care before aging out are not eligible for coverage as part of this group, but may be eligible as a different eligibility group. - Clients will need to re-verify yearly after age 19.
