Supported Living Services For Adults With Disabilities (HART-SUPPORTED LIVING) | Kentucky Department For Aging And Independent Living

Offers grants to be used towards the payment/purchase of one-time and/or ongoing supports for individuals with disabilities that supports their ability to remain in their home. Examples of supports that a grant may be requested for include home and vehicle modifications, respite care, personal care services, adaptive and therapeutic equipment, etc.

Application process

Applications may be requested from the Regional Supported Living Coordinator or downloaded online. Completed applications must be submitted by mail, email, or hand-delivery to the Regional Coordinator no later than April 1st of each year.

Eligibility

Open to any person with a disability who is a resident of Kentucky or whose family or guardian is a resident of Kentucky. The definition of disability used for this program is: a physical or mental impairment that substantially limits a major life activity such as caring for oneself, performing manual tasks, walking, seeing, hearing, speaking, breathing, learning, and/or working.
