Fees
  • Accepts Ohio Medicaid, Medicare and some private insurances.
  • Co-payments will be collected at the time of the office visit. If need to make other arrangements regarding co-payments, call the office prior to appointment.
  • Patients may make payments with a personal check, credit card, debit card or cash.
  • Offers sliding fee scale based on income and family size.
  • No patient shall be denied service due to inability to pay.
