Required documents
Proof of citizenship such as a birth certificate, social security card, or voter registration card for all household members *Proof of gross income for the past 30 days for all household members *A copy of the current electric and gas bills *Proof of Disability if applicable *Disconnect notice if applicable

Application process
Apply online OR call for an in person appointment, walk ins are accepted on a first come first serve basis. Appointments are strongly encouraged. OR by mail by sending filled application and required documents to Energy Assistance Programs, P.O. Box 1240, Columbus, OH 43216. Blank applications can be found online or in person
