Eligibility
Persons living in institutions (nursing homes, rehabilitation facilities, hospitals) who wish to return to community living. Must apply for Ohio Dept. of Medicaid’s HOME Choice Program (LEAP staff can assist with application process) and be referred by National Church Residence or be referred by an insurance provider (CareSource, Buckeye, United, AmeriHealth Caritas etc.)
