Eligibility
To qualify for HCAP:
-Must be a resident of Ohio.
-Must not be currently enrolled in Medicaid.
-Must be a individual or family at or below 100% of Federal Poverty Income Guidelines or be a current recipient of the Disability Assistance program.
Required documents
Completed application. Must submit records regarding bills and income for the three months prior to service.

- Patients must complete an application, apply and have their eligibility approved to receive free care under the HCAP program.
- Patients must submit an application within three years of receiving a statement for the specific services.
