Benefits Check-up | Cuyahoga County Division Of Senior And Adult Services
Offers one-on-one assistance to help individuals determine which private, public, state and local benefits they may be eligible to receive. Includes benefits such as Medicaid, Medicare Part D, SSI, SSD, SNAP (food stamps), HEAP (LIHEAP), Golden Buckeye Card, Exemption Prescription Assistance programs, housing and others. Will assist clients with filling out and submitting applications and forms and provides follow-up with client. Visits seniors and adults with disabilities in the home, where they provide client with combined utility assistance application, assist client, to fill it out, mail it, and follow up as needed regarding LIHEAP. Makes presentations to senior groups regarding benefit programs and provides assistance completing applications.

Open to Cuyahoga County residents outside the City of Cleveland who are age 60 or older, or adults (at least age 18) with disabilities. Presentations available for groups of seniors and adults with disabilities in the County (outside Cleveland city limits).
