- Serves Summit County youth ages birth through 21 and their families. - Youth has direct involvement with two or more local or government agencies and require coordinated care, OR - Are at risk of out-of-home placement due to escalating behaviors requiring repeated hospitalizations or interventions, OR - Have difficulty accessing services, supports and/or require assistance with cross-system navigation.
