Eligibility

To ensure the health and safety of visitors and staff, TCN is providing tele-health that allow for proper social distancing as an alternative. Anyone in the State of Ohio can participate.

Application process
To register a registration packet must be completed prior to contacting the office at 937-374-6002 . Visit their website at www.tcn.org and complete your packet. To attend a virtual walk-in clinic all you need is access to a smart phone with unlimited data or a tablet/computer with access to wireless network or high speed data.
Fees

The billing department will notify you of your cost of service.
