Refers individuals with aging and/or disability related issues to appropriate available services in the community to ensure that individuals' needs are fully met. Helps individuals find help for housing, transportation, in-home care, long-term care, and other health and wellness services including benefits screening. Helps individuals apply and be assessed for services such as PASSPORT and the Assisted Living Waiver.
