Application process
Phone for registration. Intake Associate will collect necessary information and will request an eye exam report dated within 6 months prior to application. After report is received, client will be registered and assigned a Caseworker to arrange for service.
Fee
Sliding fee scale - no one is denied service because of inability to pay.
Eligibility
Open to anyone who has an eye condition that limits functioning and is interested in this type of training.
