Individuals who are homeless, have been homeless in the past year, or who live in temporary supported or other eligible housing (including the YMCA) and have no other primary care physician.
Required documents
Proof of income and current medication containers. Those with insurance should bring it.

Application process
Telephone for appointment. Those without access to a phone can schedule their appointments with the scheduling receptionist at the front desk. Walk-ins are accepted within capacity.

Co-pay for those with Medicaid, Medicare or other insurance. Sliding fee scale for those without insurance.
