HOSPICE - IN HOME | HOSPICE OF THE WESTERN RESERVE

Provides pallitive pain and symptom management, care, and emotional support for persons with a life-limiting illness (life-threatening or terminally ill) and their families. Provides supportive services such as bereavement counseling, legal assistance, social work, art and music therapy, and spiritual care. Offers specialized AIDS and pediatric care teams. Respite care is provided for client's families.

Application process

Phone 24 hours a day, 7 days a week for enrollment information and to schedule an in home assessment. An assessment will be done to see if the person is appropriate for this program.

Fee

Fixed fee but no one is denied service because of an inability to pay. Third party payment accepted: Medicaid; Medicare; Private insurance.

Eligibility

Other eligibility may apply, but generally: Adults 18 and older who are terminally ill (life expectancy 6 months or less); Youth under 18 who are terminally ill (generally have a life expectancy of 1 year or less) or have a life-limiting diagnosis and are in a state of decline. Serves Ashtabula, Cuyahoga, Geauga, Lake, Lorain, Medina, and Summit counties, with outreach to Portage county.
