Graduates of an accredited College of Optometry who intend to practice optometry in Ohio, optometrists licensed in the state Ohio needing a renewal, or optometrists licensed in another state who intend to practice in Ohio.
New applicants: Completed application, photograph, final and official transcript from an accredited College of Optometry, Cardio Pulmonary Resuscitation (CPR) training certificate, and National Board of Examiners in Optometry (NEBO) scores. Renewals: Completed application and continuing education courses. Those born outside of the United States will need proof of residency such as a passport, visa, or green card. Those licensed in another state will need to show proof of good standing in that state by submitting appropriate form (varies by state).
M-F: 7:30am-4:30pm.

Preferred method is through website for information and applications. Applications and renewals may be submitted by mail only. Formal approval for new applicants will take place at board meeting, held five to six times throughout the year. Renewals are approved automatically after continuing education is verified. Phone to register for legal seminars M-F: 7:30am-4:30pm. Class times vary throughout the year. Web site provides licensure and related information anytime and contains a list of continuing education courses.

Fixed fee for licensure and annual renewal. Law seminar is free. Fees for continuing education courses depend on organization offering the instruction.

Grants licenses to optometrists, handles renewals, and enforces continuing education. Provides a seminar on the Ohio laws and rules of optometry, a requirement for licensure. Optometrists need to complete 25 hours of continuing education yearly to renew license; preferably October through September of each year.
