Call to complete intake. Transportation available to and from center.
Fees
Accepts private insurance, Medicaid, Medicare, managed care, private pay (cash, check, or credit card).
Service area
Wayne County, Ohio
Day center for seniors providing meals, physicial activities, field trips, social interactions, stimulating games, arthritis relief exercises, memory activities, caregiver support, medication assistance, bathing assistance, feeding assistance, and assistance with toileting.
Staffed by caregivers, certified Registered Nurses (RN), and State Tested Nurse Assistant (STNA).

- Recommended that clients bring a change of clothes to keep at facility. If taking medication while at facility, required to bring prescription bottle to keep at facility. Also bring personal hygiene items, if needed, such as depends or pads.
