  • Serves low- and moderate-income residents of Avon and Avon Lake residing in ZIP codes 44011 and 44012.
  • Household must be at or below 200% of the Federal Poverty Guideline Level OR household must have experienced a 50% loss in income due to job loss or medical issue.

Required documents

Will be requested as needed and include:
  • Photo ID.
  • Proof of address.
  • Copies of utility bills.
  • Proof of income.
