Path (PROJECT ASSISTANCE IN TRANSITION FROM HOMELESSNESS) | Frontline Service

Provides outreach and assessment services for severely mentally disabled persons who are currently homeless. Offers outreach to the streets, shelters and other places the homeless gather; helps clients stabilize and make the transition into community support programs. May take client to shelter, detox, or other community resource.
