AFFORDABLE CARE ACT (ACA) HEALTH INSURANCE MARKETPLACE ASSISTANCE | NEIGHBORHOOD FAMILY PRACTICE
Provides Certified Application Counselors (CAC's) who aid individuals in navigating the Affordable Care Act Marketplace. Assesses individual health insurance needs, educates about basic health insurance issues, provides information on health insurance plans, helps individuals register and obtain health insurance through the Marketplace, and screens for Presumptive Eligibility - Medicaid and Medicaid eligibility. Can also assist with Medicaid renewals.

Fee
No fees for Application Counselor services. Insurance plan cost will vary depending on the kind of plan one chooses to purchase through the Marketplace. Those with income between 100-400% of the Federal Poverty Line may be eligible for financial assistance programs designed to help pay for an insurance plan.
Eligibility
Open enrollment is annually November 1-December 15. But some may qualify for “special enrollment” if they have had a "qualifying life event," such as starting or leaving a job, certain changes in income, and/or changes in family size (for example, if they have marry, divorce, or have had a baby.) Note: Individuals must act within the first 60 days of the "qualifying life event" to qualify. Those not eligible for Medicare and who are curious about their public health insurance options. Individuals who have an income through 138% of the poverty level should consider getting screened for Medicaid before contacting the Marketplace or an assister.
