All Ohio residents age 60 or older, as well as adult residents age 18-59 who have been certified as having a permanent and total disability.
Application form, proof of age, proof of Ohio residency, proof of disability if applicable: Social Security award letter, Medicare card, copy of any documentation certifying permanent and total disability that has been issued by a State of Ohio Dept. (MR/DD, BWC etc.) or Federal or State of Ohio public employee retirement system.

During the month of their 60th birthday, cards will be mailed to Ohio residents who have an Ohio driver's license or a State of Ohio ID card. Those not receiving a card, needing a replacement card, or wanting information may phone M-F: 8:00am-5:00pm or apply in person at most libraries and senior centers. Information also available on the Web site anytime.

Provides a discount program for Ohio's older adults and permanently disabled residents. Participating businesses provide special offers to cardholders, including a percentage off purchases, flat-rate discounts, free or discounted products and services, and more. Businesses participate voluntarily and set the details of their offers.
