Eligibility

Open to individuals in Hamilton and Clermont Counties who are at or below 300 percent of the Federal Poverty Level without insurance or with insurance with unaffordable copays.

Note: Individuals actively on Ohio Medicaid insurance plans are ineligible to receive assistance.

Application process
Registration requried. Patients must complete an in-person certification process before receiving medication. Patients may certify for the pharmacy program by walk-in, appointment or physician referral.
