Individuals must first meet membership requirements of clinic at which they apply. Must also be Cuyahoga County residents, age 19-64. Must be uninsured and ineligible for government health insurance programs. Family income (where "family" refers to a single individual or married couple, with or without children) at or below 200% of the federal poverty guidelines.

Description

Offers a membership program through several community clinics that gives eligible clients of those agencies discounted access to specialists at participating local hospitals. Client will still receive all primary medical services through their clinic. Discounts only apply to necessary specialist visits as referred by the client's clinic-based general practitioner. If already a client of one of the clinics, can enroll in the plan through that clinic at any time. All new clients will be automatically eligible as part of the clinic enrollment process.
