For proof of income to be considered for sliding fee schedule, one of the following documents will be accepted: 1) one month of pay stubs 2) tax return, W2 or 1099 form from the most recent tax year 3) Social Security benefit verification letter.
Danville Location - East Main Street: Mon-Fri 8am-4:30pm. Knox County Community Health Center: Mon-Fri 8am-4:30pm Open 7:30am-6pm Third Tuesday of each month. Moore Family Practice - 206 South Mulberry Street - Mount Vernon Mon-Thu 8am-4:30pm Fri 8am-2pm. Walk-In Clinic - 207 West High Street - Mount Vernon (inside Foster’s Pharmacy) Mon-Fri 8am-4:30pm. Closed for all federal holidays. Same-day appointments available, walk-ins welcome.

Out-of-pocket expenses can be reduced depending on income and family size. Costs are significantly lower than going to ER or urgent care. Significant cost reduction for any Lab work and prescriptions. Regardless of payer source, NO ONE is turned away. Nominal fee for medical services is $20.

Services include medical, dental, addiction treatment, mental health counseling, as well as women's health, well child visits, exams for work, school and sports MEDICAID Patients welcomed.
