Telephone. Appointment required. Call and leave a message. Clients should leave name (spelled out), date of birth, why they would like to be seen and a contact number. Clinic Coordinator will call back to schedule an appointment for Wednesday evening.

18 and over. Must be at or below 200% Federal Poverty Level. Must be uninsured. Cannot be eligible for Medicare or Medicaid.
