Application process

If someone suspects they were exposed to the disease or have symptoms consistent with the disease, they should contact their primary care provider for testing. Information on the disease is available online. Those with clinical questions, need for vaccine, treatment or testing, should contact their health care provider. Widespread vaccination is not recommended at this time. Callers should contact the Board if they need to report test results, concerns of exposure, potential outbreak settings (i.e. Callers know of multiple cases that attended a party, event, household, etc.), or arenot sure of isolation or quarantine requirements.

Eligibility

Serves residents of Cuyahoga County, with the exception of the City of Cleveland. Distribution of vaccines or treatment must be requested by a health care provider and approved by a statewide authority.
