Application process

Cards are automatically sent by mail to adults 60 years and older with a valid state ID. Individuals who do not have a current license or State ID, as well as people under age 60 who are eligible due to disability, or who otherwise do not receive their card automatically, may apply for a card at most public libraries and senior centers.

Residents of Ohio who are 60 years and older and individuals 18-59 who have been certified as having a permanent disability.
