Application process
  • Call the Ohio Benefits line or visit the website at https://benefits.ohio.gov for more information, to apply for benefits, or update information. Must submit a completed, signed, and dated application.
  • Renewal Process: Yearly renewal packets are mailed to the client's home. Signed packets can be returned through the mail, in the drop-box in the front of the building, or by fax to 440-323-3422. This process can also be completed by phone by calling 1-844-640-6446 or online at www.benefits.ohio.gov.
  • Supporting verification documents for both the initial application and renewal process can be mailed to the office, dopped off, faxed 440-323-3422, or emailed to [email protected].
