Application process

Applications can be submitted via ssp.benefits.ohio.gov, emailed to [email protected], faxed to 1-886-351-8292, or deposited in the curbside drop-off box located in front of the main entrance of the building (dark green box). - NOTE: Clients who are currently on this program should make sure to reapply before the 12 months of service ends in order to remain eligible for this service.

Fee

Offers a sliding scale for co-payments based on household income and family size.

Eligibility

- Caregiver must live in Summit County and be employed; enrolled in school; or receiving education necessary to maintain employment, certification, or licensure. - For initial application, income must be 200% or less of current Federal Poverty Limit (FPL). - Reapplication is required after 12 months of assistance - current recipients may have income up to 300% FPL when reapplying. Eligibility is re-determined annually. Changes in household composition and unreported income could all result in changes to benefit amount. Recipients are responsible for reporting these or any other changes within 10 days of occurrence. Receiving more than a household is entitled to could result in overpayment penalties or charges of fraud.
