Eligibility
Immunization clinic-Serves residents of City of Canton age 18 and under. Children under age 18 must be accompanied by a parent/guardian. No one will be turned away for inability to pay.
Travel clinic-no residency requirement.

-Immunization/shot record.
-Medical or insurance card, if applicable.
-Parent photo ID.

Immunization clinic by appointment (walk-ins welcome):
Monday: 8:00am to 3:45 pm,
2nd Thursday: 4:00pm to 6:45pm.
Travel clinic by appointment only:
Thursday 8:00am to 11:15am.
