Application process
Call to make an appointment. New patients need to complete new patient forms. For Patient Transportation Service, call 419-522-6191.
Fees
All insurances accepted including Medicare and Medicaid. Sliding fee is available for qualifying patients. Sliding fee means the bill may be reduced, not free. Please be prepared to make the minimum payment required at every visit.

Provides Gynecology and Obstetric services. Offers to any Third Street OB/GYN patients free early gender reveal at 14 weeks. Also offers free pregnancy tests.
