Parents and professional can apply for all ages and disabilities. Must own an iDevice and be able to explain the need for the program.

Diagnosis letter
copy of most recent licensure if being requested by a service provider
documentation demonstrating how app will be implemented
name, cost and developer information for the app
250-500 word explanation of why the app is necessary and why purchasing it is not possible.
