West Side Senior Services And Nutrition Program - Morning, Afternoon | Salvation Army Of Greater Cleveland

Seeks to help older adults remain in an independent living situation by offering supportive services including on-site hot meals and home delivered meals. Clients can choose to have one or more of these services. Diabetic diets can be accommodated for home delivered meals with prior arrangement.

Application process

For in-home services, a social worker will visit the home and conduct an assessment before services can begin. When necessary a nurse will also conduct a medical assessment. Phone for information, to make an appointment for an assessment or to make a reservation for the on-site hot meal. Reservations for the on-site meal must be made by 11:00am on the Wednesday of the previous week. There is a waiting list for the home delivered meals.

Fee

Clients are asked to pay what they can but services are free for those unable to pay.

Eligibility

Adults 60 years or older, low-income (generally 150% of poverty but may be higher), and live on the near West side of Cleveland. For home delivered meals must live in the following service area: 44111 and 44135 from W 117th St. to W 140th St. and Brookpark to Triskett.
