Eligibility
  • Serves children and young adults with severe to profound intellectual and developmental disabilities (generally, birth through age 35).
  • Cannot accept persons who are non-ambulatory but can accept those with a tracheotomy (trachea) tube.
Required documents
Will be requested as needed.

24 hours a day, 7 days a week.
