Description

OPTIONS is a program that links people in need with dentists who have agreed to treat approved patients for reduced fees. Step 1: Call toll free 1-888-765-6789 for an application. Step 2: Fill out the form and include proof of your family income; such as a W-2 form, your last tax return, or your two most recent pay stubs. Step 3: Mail the form and proof of income to the address on the form. Then an OPTIONS coordinator in your area of the state will call a dentist near you to see you for an exam. The coordinator will call you with the name and phone number of the dentist so you can schedule your first dental visit. Final acceptance into the program is made after the first dental exam. Most OPTIONS patients qualify for reduced fee care and must pay those fees at the time they receive care. Some people who are elderly or disabled and who have few resources may have their care donated by OPTIONS dentists. The dentist you see will decide what treatment you will need. Services may include: an exam, x-rays, cleaning, fluoride treatment, sealants, fillings, extractions, dentures or partials.
