Offers a Medicaid State Plan service to support those in the community diagnosed with severe and persistent mental illness. Assists with maintaining Medicaid eligibility. Assigns a Recovery Manager to collaborate with behavioral health agencies and oversee activities such as medical appointments, social interaction, and living skills.

Eligibility

- Serves individuals with a diagnosis of severe and persistent mental illness. - Must reside in Summit, Portage, Stark, or Wayne county. - Household income must be $2,199 per month or less. - Must be age 21 or over. - Must not currently reside in a nursing home, hospital, or similar setting. - Must be determined disabled by the Social Security Administration.

Plans, funds, and coordinates services for older persons and persons with disabilities in Portage, Stark, Summit, and Wayne counties to live with dignity and choices in their homes and communities for as long as possible. Administers PASSPORT, MyCare Ohio, The Assisted Living Waiver, The Ohio Home Care Waiver, Care Coordination, Elder Rights, Family Caregiver Support Program, Nursing Home Placement Assistance Program, and other services designed to help older adults and individuals with functional limitations maintain independence. Provides funds for services administered by other community agencies including client finding, specialized information and referral, transportation, home-delivered meals, medical treatment, personal care, respite care, safety monitoring, health assessments, congregate meals, recreational activities, and adult day care.
