Women residents of Ohio who are pregnant woman or have had a baby within the last two months, who is a US citizen or who meets certain citizenship requirements and who meets Federal Poverty Income Guidelines. See https://www.benefits.gov/benefit/1610 for more information.
Required documents
Proof of income, social security card, birth certificates, marriage certificate, custody papers, if applicable, proof of child support or SSI payments, immigration documents - if you or someone in your household is NOT a US citizen and applying for Healthy Start, proof of alien status is required.
