Application process
Walk in. Do not eat or drink anything but water for 9-12 hours prior to event if planning to do blood test.
Registration required for mammogram and vaccines. For mammography screening registration or questions, call OhioHealth at 567-309-2512. For vaccine registration or questions, call the Health Department at 419-282-4357.
Fees
Cash or check for most services, with sliding fee scale available for some services. Will also accept private and public insurance plans for some services.
