Eligibility
Must be a woman who is pregnant OR lactating with a baby up to one year of age, OR be postpartum for up to six months after the termination of a pregnancy by birth, miscarriage or abortion. Those younger than 18 do NOT need parental/guardian permission. Children are eligible up to fifth birthday. Eligible individuals must live in Ottawa County, must be at nutritional risk as determined by an agency professional using the Federal Nutritional Risk Priority System and must meet income guidelines. Income eligibility guidelines are up to 185% of the Federal Poverty Guidelines. Number in household includes unborn children (Example: A pregnant, single woman is a household of 2.) Definition of income does NOT include household assets. Individuals receiving Medicaid, Ohio Works First or SNAP benefits automatically meet income eligibility.
