Referral from primary care physician or nurse practitioner. Insurance card and photo ID. Staff can assist client in obtaining referral.

Application process

Phone M-F: 7:30am-5:00pm or leave message (which will generally be returned in 1-2 business days) for information and to register for counseling or a support group. In-person services are held in multiple University Hospital sites.
