Eligibility

Serves individuals who are not eligible for services through another payer, such as Medicaid, Medicare, insurance, or hospice. Open to residents of Hamilton County age 60 and older who require assistance with certain activities of daily living. Eligibility for specific services is determined by a care manager during an in-home assessment.
