-Primary Care Hours: Monday 9:00 am to 7:00 pm, Tuesday - Friday: 9:00 am to 5:00 pm.
-Pharmacy Hours: Monday - Wednesday, Friday: 8:00 am to 5:00 pm, Thursday: 8:00 am to 7:00 pm.
-Behavioral Health Lab: Monday - Friday: 8:30 am to 4:30 pm.
-Lab Hours: Monday: 10:30 am to 6:30 pm, Tuesday - Friday: 8:30 am to 4:30 pm.

Application process
New patient visits are available on a drop-in basis, both online and in person. Patients may also schedule an appointment online, by phone, or by live chat on the website during business hours.
Fees
Medicaid, Traditional Medicare, and some Medicare Advantage Plans are accepted, as well as a sliding fee scale.
Does not process private insurance claims.
