Original prescription and the application (which can be completed online or over the phone.) Income documentation is NOT required - the signature on the application or verbal verification is accepted as attestation that the information is true and accurate.

Can enroll online or by phone. Applications can also be printed off the website and mailed or faxed in. Once approved, medication ships within 7-10 days. Telephone Call Center can assist with any questions.
